Primary Health Networks

PHN is a not-for-profit organisation who partners with community organisations, health care providers, hospital and health services, and state and federal governments to improve primary health care services in our communities. PHN works within the Darling Downs and West Moreton region to better understand, support and improve the health of people living in our region. PHN brings together GPs, general practice staff, allied health professionals, hospital and health services, Aboriginal Medical Services, community service providers, Elders and community members to identify the services that are needed the most, and then fund programs and services based on these needs.
PHN also support primary health care providers, like GPs, practice managers, practice nurses and allied health professionals, to improve the care they provide to their patients.
PHN offers two access points for people at need or at risk of Chronic Health Conditions and/or Mental Health.

Chronic Conditions

PHN is working with primary health care providers to support people who are living with chronic conditions.
These services provide early intervention, prevention and care coordination support for people living with eligible chronic conditions and/or risk factors.
Cost of Program
PHN have funded allied health providers across our region to deliver services guided by the National Strategic Framework for Chronic Conditions. There are no out-of-pocket expenses with a GP referral.
How to Access
Patients must have exhausted all other funding options prior to using Darling Downs and West Moreton PHN Chronic Conditions Program funding. This includes their private health insurance, Department of Veterans’ Affairs, Medicare and NDIS funding AND have a diagnosed chronic condition or risk factor, specifically:
